New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. 

You will need to complete a registration form. You can either print and complete the registration using the following link PRF1-050723.pdf (gp-registration.nhs.uk) 

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date. If you are taking regular medication, you will need an appointment with our practice pharmacist to discuss getting them onto repeat.  Please make sure you have enough medication from your previous surgery to last you until your appointment with our pharmacist.  

We will require new patients to read our policies and sign to confirm you understand them. We also require you to provide proof by way of a recent utility bill. We also require one form of photographic ID. These forms will need to be completed before we can complete your registration. 

 

Medical treatment is available once you are registered at the Practice, please allow 48 working hours for registration to be completed. Please contact reception for further information.
